One-page form used to record burials at the Millington site during the 1938-1939 WPA work. The form would have been typed up in the field (or perhaps that night) by Lehmer, the supervising archeologist. Compared to modern recording forms this one seems primitive, but the WPA work at Millington and other sites across the country represented a great step forward toward more systematic and thorough archeological field documentation.
